1st I binned it trying to brake on gravel in a lefthand corner
2nd The back slid out on me in a lefthander - too cold to get grip
Both are directly attributable to me pushing the bike beyond what was safe in the conditions.

First Bike
My first bike was a 1963 Triumph 200 Mountain Cub which I purchased in 1966. I'm 5' 10'' and 165 lbs.

2 seasons 91 to 92 then quit and now getting back in the saddle
one accident - I was passing a car at around 50mph when he decided to make a quick left turn. He sideswiped me but not serious. Cut up my ankle and locked my back breaks and skidded for what felt like 100ft. Luckly I didn't go down. I just rode out the locked breaks.
