There was a loose nut on the handlebar.

I'm not counting falling over in parking lots as accidents. That's just being dumb at low speeds.
My one accident was in '94 when I hit a patch of gravel going around a corner. It was a right turn and in town, so it was slow with no major damage. I already had the scenario worked out in my head, so when I felt the bike going down, I just hopped off. There I was sitting in the intersection watching the Magna motor off into some shrubbery by itself. Broke the windshield and one turn signal and my butt was lightly bruised from the landing. I have serious respect for gravel and corners to this day.
