Mike, just roll your sleeves up and customize the smilies page. With a minor bit of work you could get the most commonly used smilies at the top of the page. Couple database schema changes (maybe) and a slightly different SQL query and we're set.
Of course this is the kind of stuff I do for a living and for fun.
(edit) Haha, I find it hilarious a smilie mocking a stupid idea for a smilie made it here from Something Awful.
