My hands are probably much bigger than yours, but continuous practice has made the technique comfortable. I do this when commuting on interstates (as I posted before in this thread). Thumb on the throttle housing, second and third fingers resting on the brake lever, fourth and fifth fingers holding the throttle. I remember when I first started, it wasn't comfortable at all. Now it's easy and automatic. I think doing it all the time stretched the tendons in my right hand.
