Help my school project, answer this poll about accidents

I'm doing a report about how to reduce motorcycle crashes. I would like to use a survey of actual riders, but I need a good number of responses. If I get fifty responses total, I'll be thrilled. So please answer away.

Good job! This is basically what I was trying to do with my accident poll, but was an idiot and let out the "no" option. Yours is much better!

I could honestly say I have had more than one incident in a couple different categories or I avoided them by being fairly paranoid when i am on my bike. Lot's of times when i stop i think back to how many times things could have gone wrong if i had just been paying a little less attention to my surroundings.

Sweeping left hander carrying too much speed, followed by braking on gravel, followed by sliding across gravel on face

Followed at the start of this year by loosing traction on a tight lefthander. Backwheel just slide out from underneath while I had her right over. Greasy roads.

Accepting I was late and not pushing the bike beyond the limits of the road to try and make up time would have saved a lot of trouble and some medical treatment.

Yeah, by the way, if more than one of those first four things happened to you, choose the one that has happened most frequently, or the one which was worst. And feel free to add details and info about other near collisions that you couldn't put in the poll. All info is welcomed and wil be helpful.

Yes. the driver made a left turn or nearly did right in front of me.

I have had this happen to me twice. Once on my bike where I hit the car at about 70kp/h which wrote me and the bike off. Second time was at work where a taxi did a u-turn and took me out.

No. I've collided with another vehicle but it was my fault.

I ran up the back of a car on my old Katana which had average brakes at best. The traffic stopped suddenly and I was unable to stop in time so I attempted to dive between the cars in front but as I stopped my left hand and bar smashed into the guys tail light.

No. I've only crashed by myself.

I lowsided my first bike going around a tight left-hander on cold tyres, lost the front end and slid down the road.

I highsided my last bike due to a little too much accelleration and wrote it and myself off.

Also, I have had too many instances of cars changing lanes without seeing me or coming around a corner into my lane almost causing a head-on to mention.

Good times.

Vehicles changing lanes, more than a few times. Big rig on the interstate, too.

To be honest, these are NOT limited to motorcycles so please be careful not to make the report sound as if they do. I've had (we all have, I'm sure) the exact same things happen while in the car. I believe more often than not that when someone says "I didn't see the motorcycle" it could be translated as "I didn't bother to look before I decided to change lanes". I make a point of looking for the person's eyes in their mirrors, and in the half dozen or so near misses I've had, every single driver never looked before chaning their travle path.

Hmm.
When I was hit by the taxi, the driver turned his head looked me square in the eyes and turned his car straight into the side of me. He later said to the copper "I did not see him".
I watch to see them look for cars, but it does not mean they see me.
