The whole idea is that maggots will only eat rotting or septic flesh. The stuff that needs to be cut out no matter what. So if they only eat the bad stuff then they can be a LOT more selective then a scalpel ever could. After they stop eating and begin to gestate they are removed and new ones are added. This continues until all the dieing flesh is gone.
