For me out of that list would be "Not paying enough attention" as well. There is so much to look at and do while riding at bike that you can easily miss something minor some of the time, and sometimes something more major (like running out of paved road to gravel road at 110 km/h!).
A rider will develop at speed at which they can be confident and that they can rubberneck at, for me, it's about 80-90km/h. The faster I go than that the more focused where I am heading I gotta be. And thus I "see" less around me.

Have to agree with "not paying attention" and; that's not necessarily the motorcycle rider but everyone else around you in cars. With the explosion of cell-phone use over the past few years as well as a littany of other driving distractions; it's more important than ever that the most vulnerable (us motorcycle riders) take even more care to be aware of everything going on around them at all times. Never take anything for granted. It's dangerous out there people so stay alert; please!
