Did the bearings themselves fall out whole, or did the balls/needles/rollers fall out of the races?

Well, at minimum the bearings. Having not seen the bike I don't know if the races were damaged, too. Pick up some new bearings and see if they'll seat in the races without excessive effort. Hopefully the failed bearings were causing the drag on your brake rotor.

Bingo. That's not a normal condition. I'd get new bearings installed. Riding it in that condition will wreck your hub and that's gonna cost ya....
