Time for a new chain, as well as sprockets if they are worn.
It is always best to replace sprockets and chain at the same time, otherwise the new parts will wear out faster, leading to earlier replacement intervals.

You need a new chain and sprocket.
The chain has stretched out -between each link- therefore the spaces between links no longer corresponds perfectly with the spaces on the sprockets. This has led to wear on the sprocket - so once you buy a new chain, it will not fit properly on the old sprocket.
Hence new chain and sprocket - probably a good idea to fit a new front sprocket as well.
To break a chain (and usually to fit a chain) you will need one of these
